Do I Need a Prescription?
California grants “Direct Access” to physical therapists’ services, meaning you do not need a physician’s referral to see Mary Ruth Velicki MS, DPT. However, if you plan to submit a claim(s) to insurance for your visits, we encourage you to ask your insurance company before your first visit if a referral or prescription is necessary in order for them to process your claim.
Clients pay in full at the time of service. Sliding scale available.
Holistic Bodywork and Physical Therapy does not participate with any insurance providers, including Medicare. and are given a statement to self-submit to their insurance provider which includes proof of payment, along with the diagnostic (ICD-10) and treatment codes (CPT codes) for each session.
If you wish to submit your claim, Holistic Bodywork and Physical Therapy, PC encourages all new clients to contact their health insurance company or other reimbursement services prior to the first visit to determine the their “out-of-network” physical therapy benefits. Your payment includes your treatment session time and the generation of a super-bill. If you would like me to contact your insurance or others or prepare other paper work for you, the fee for this service is $40.00/ hour.
Unfortunately, due to Medicare regulations, clients over 65 cannot receive reimbursement from their insurance provider, secondary or otherwise, for this service.
Please provide 24-hours notice if you need to cancel or reschedule an appointment. This allows the opportunity for someone else to schedule an appointment. If you are unable to give us 24-hours advance notice you will be charged the full amount of your appointment. This amount must be paid prior to your next scheduled appointment.
If you arrive late, your session may be shortened in order to accommodate others whose appointments follow yours. Depending upon how late you arrive, your therapist will then determine if there is enough time remaining to start a treatment. Regardless of the length of the treatment actually given, you will be responsible for the “full” session.
